Enrolling as a patient is simple

Please have two forms of documentation ready:

  1. Valid proof of identity or photo ID (e.g., passport, driver’s licence, etc.)
  2. Proof of eligibility to enrol
    • If you’re a New Zealand citizen, your NZ passport or NZ birth certificate is ideal
    • If you’re not a NZ citizen, you’ll need to provide a copy of your passport and your valid visa (e.g., work or resident visa). Note that only certain visa types meet the eligibility criteria. Please refer to the Guide to eligibility for public health services for the full details.

Everyone aged 16 and over must complete their own individual patient registration form.

When asked for a location on the registration form, please select Phoenix Health Hub – Rolleston as your preferred clinic.

While the new clinic prepares to open its doors on 19 May 2025, you can be seen by a practitioner in person at Phoenix Health Hub Moorhouse in central Christchurch, or via telehealth appointment with your preferred clinician. Bookings essential.

You also have the option to book a virtual consult through our trusted telehealth partner, Practice Plus.

Patients enrolling at Phoenix Health Hub Rolleston will enjoy full access to services across all Phoenix clinic locations in Canterbury.

Patients at our Rolleston Central clinic are serviced by experienced practitioners to ensure timely and reliable access to care. Our team can deliver high-quality care to manage both acute and long-term conditions.

Off Work Certificates – Generalised Illness
  • Enrolled Patients: $30 
  • Casual Patients: $35 
  • Covers up to 7 days off work
    Please note: This does not include ACC Extensions, university paperwork, or WINZ/Disability allowances.
Blood Tests Available after being requested by your GP, or if you have received an SMS or email reminder. 
Prescribed Medication Injections   Administer injections prescribed by your GP, including B12, Depo/testosterone. 
Please note: This excludes steroid injections 
Sexual Health Consult  Comprehensive checks including swabs and blood tests, with friendly, confidential support
Cervical Screening   Choose between a self-swab or a clinician-collected sample.
From $25 and free for some patients. *
Vaccinations as per the NZ Vaccination schedule (Including Flu vaccine)  We provide vaccinations as part of the New Zealand schedule, including the annual flu vaccine.  

Please note we are not currently offering travel, work-related, or NZ Defence Force vaccinations. Our Rolleston clinic will not have Flu vaccines available till April 2026. Care Plans Work with our team to develop personalised, goalbased plans, including Advanced Care Plans. 
Smoking Cessation Support  Supportive discussions and referrals to help you quit smoking. 
Lifestyle Support  Guidance around healthy eating and exercise to support your wellbeing. 
ECG (Heart Rhythm Scan) Available after a GP assessment and referral. 
Annual Diabetes reviews Includes foot checks, lifestyle discussions, and organising retinal screening referral 
Cardiovascular Risk Assessment For those aged 45-75**
**Please note some patients may benefit from starting screening from age 40 – contact our team to discuss if this is suitable for you** 
Wound Care & Dressings We provide dressing changes for both ACC and non-ACC related wounds (including suture removal.
Liquid Nitrogen Treatment Available for warts, verruca’s, skin tags without a GP appointment.
Other skin lesions (such as moles) required GP review first. 
ACC Injury Care We can manage simple new ACC injuries.
Excludes injuries requiring imaging (X-ray) and injuries to the neck, back, head, or eyes. 
Visual Acuity Test Check your vision quickly and easily.
MiniACE Cognitive Assessment  A short test designed to assess short-term memory and memory recall. 
New Patient Appointment  Completed at your first GP visit to review your medical history.

GP Clinic Fees

All NZ citizens and permanent residents receive Government funding to support your general practice healthcare needs. Appointments for tamariki under 14 are free. Our GP Clinic also offers reduced fees for patients with Community Services Cards and High User Health Cards, as long as they are provided at the time of your appointment.

If you cancel or reschedule your appointment between 3- 24 hours prior to the scheduled appointment time, we may charge a cancellation fee. If you cancel or reschedule with less than 3 hours’ notice a 100% cancellation fee may be applied.

Failure to attend a scheduled appointment may attract 100% fee.

If you are more than 5 minutes late to your scheduled appointment, we will need to cancel the appointment, and 100% cancellation fee may apply
