Rolleston – Patient Enrolments Now Open

We are now accepting patient enrolments: ENROL HERE

If you’re looking to enrol with our new Rolleston clinic, please choose the location “Phoenix Health Hub – Rolleston” from the list.

If you’re filling out the enrolment form and the “Find address in eSAM” feature just keeps loading, you can skip that field and enter your address manually.

Here’s what to do:

1. Leave the “Find address in eSAM” field blank.
2. Manually type your address into the fields below it (e.g. street number, street name, suburb, postcode, etc.)
3. Ignore the “Address not validated” warning at the bottom of the section—it won’t stop you from moving forward.

Once all required fields are filled in, click “Next” to continue.

Access to the Patient Portal will again be available beginning Monday 21st of April.
