Our GP clinic Phoenix Health Hub is a community focused general practice working with you to optimise the everyday health of you and your whānau.

Our friendly team of medical professionals provide quality health care, advice and support for patients of all ages in our community. We take a patient-centred approach to ensure you and your whānau receive the very best care, tailored to your health needs.

We offer in person or telephone consultations for enrolled patients and can see casual patients for all medical services at the GP clinic.

Please note that some services are subject to availability between our Moorhouse Ave and Rolleston clinics. We recommend checking with your preferred location in advance.

Clinic Appointments

There are several ways to book a standard GP Clinic appointment (15mins) with Phoenix for you or your family:

  • You can give our friendly reception team a call between 8:30am – 5:00pm, Monday to Friday
  • If you are already enrolled as a patient, you can easily book an appointment online
  • If you are not enrolled and would like to book in as a casual patient, please call our reception team.
  • If you would prefer to pop in for a chat about our services or make an appointment, we would love to see you!

What our Nurses can do!

Our nurses are a vital force in healthcare, blending nursing and medical ability. Our Nurses at Phoenix healthcare provide high quality, patient centred healthcare services.

Medical Certificates
  • Nurse-issued medical certificates are available for up to 3 days
  • Can be issued via phone or in-person consultation
  • For certificates over 3 days, a medical review with a clinician is required
  • Does not include ACC, WINZ, or similar statutory documentation
  • Charges may apply
Blood Tests / Laboratory Tests
  • Available at Phoenix Health Hub (fees may apply)
  • Available at Awanui Labs (generally free of charge
Injections, Infusions & Treatments Administered by the nursing team, including:

  • Depo-Provera
  • Vitamin B12
  • Zoladex
  • Iron infusions
  • Some services may be funded depending on eligibility
  • Otherwise charges apply
Sexual Health
  • Full sexual health screening available via nursing team
  • Charges may apply
Cervical / HPV Screening
  • Delivered by the nursing team
  • Funded for eligible priority groups
  • Otherwise charges apply
Vaccination Programme Includes:

  • Childhood immunisations (NZ National Schedule)
  • Adult vaccinations as per schedule
  • Pregnancy-related vaccines
  • Annual influenza vaccinations
  • COVID-19 vaccinations are not provided
Workplace & Travel Vaccines
  • Arranged via a GP consultation
  • Vaccines must be pre-paid prior to ordering
  • Administration completed by nursing team
Complex Care Includes:

  • Acute care planning
  • Personalised care planning
  • Advance care planning
  • Delivered by nursing team

Liquid Nitrogen Treatment
  • Provided by nursing team
  • Requires prior clinician assessment for suitability
Lifestyle Support Delivered by nursing team in collaboration with the in-house wellbeing team:

  • Health Improvement Practitioner
  • Health Improvement Coach

Support includes:

  • Smoking cessation
  • Weight management
  • Physical activity support
  • Stress management
  • Sleep hygiene
  • Referrals to external support services as required
ECG (Heart Tracing)
  • Performed by nursing team or as directed by clinician
  • Charges may apply (patient informed at time of care)
ACC / General Nursing Services Includes:

  • Wound care
  • X-ray requests (where clinically indicated)
  • ACC reviews and follow-up care
  • Managed by nursing team
Cardiovascular Risk Assessment
  • Completed for eligible patients
  • Patients will be contacted by nursing team if eligible
  • Includes follow-up planning and referrals where required
Diabetes Annual Reviews
  • Coordinated with lab testing and GP review
  • May include nurse-led comprehensive review
  • Follow-up and referrals arranged as required
New Patients
  • Initial nursing assessment is completed for all new patients
  • Followed by first GP consultation
Cognitive Impairment Screening
  • Completed by nursing team as part of broader assessment or standalone review
  • May be included in driver licence medicals
  • Charges may apply

GP Clinic Fees

All NZ citizens and permanent residents receive Government funding to support your general practice healthcare needs. Appointments for tamariki under 14 are free. Our GP Clinic also offers reduced fees for patients with Community Services Cards and High User Health Cards, as long as they are provided at the time of your appointment.

Please note that insurance, immigration, and occupational medicals aren’t covered under standard funding, so these appointments will need to be paid for privately. If you have any questions, we’re always happy to help!

If you cancel or reschedule your appointment less than 3 hours prior to the appointment time, a $30 cancellation fee may be applied.

Failure to attend a scheduled appointment may incur a $30 cancellation fee.

If you are more than 5 minutes late to your appointment time, we will cancel your appointment, and a $30 cancellation fee will be charged.

For 30-minute appointments, if you are more than 10 minutes late, your appointment may be cancelled, and a $30 cancellation fee may apply.

We value the opportunity to care for you and appreciate your prompt attention to billing matters. Please note that accounts with overdue balances may incur a $10.00 administrative fee if payment is not received by the due date. Enrolling as a patient is simple

Please have two forms of documentation ready:

  1. Valid proof of identity or photo ID (e.g., passport, driver’s licence, etc.)
  2. Proof of eligibility to enrol
    • If you’re a New Zealand citizen, your NZ passport or NZ birth certificate is ideal
    • If you’re not a NZ citizen, you’ll need to provide a copy of your passport and your valid visa (e.g., work or resident visa). Note that only certain visa types meet the eligibility criteria. Please refer to the Guide to eligibility for public health services for the full details.

Everyone aged 16 and over must complete their own individual patient registration form.
